LaSen Operator Qualification (OQ) Plan
OQ Plan
The purpose of this document is to serve as the Operator Qualification (OQ) Plan for all Field Technicians working in the helicopter while performing aerial leak detection services on behalf of LaSen, Inc. This document also serves to establish the Policy and Procedure for the OQ process for all Field Technicians at time of hire. The following document will outline the OQ process for new hires, how LaSen Inc. maintains these OQ’s, the audit process, span of control, and will detail the OQ’s currently held by our Field Technicians.
Note – Since LaSen, Inc. is a contract organization, it is customary that LaSen, Inc. carry any OQ’s that are required and set forth by a Pipeline Operator prior to starting a project. Therefore, the detailed OQ list provided only serves as the existing framework required by our existing Operators and should not be considered all-inclusive for any future work. It is Standard Operating Procedure for LaSen, Inc. to investigate, and or update, what the OQ needs of the Pipeline Operator are prior to starting a project.
New Hires
All new employees are put through a week-long training environment in which they must successfully complete an OQ training block. This training block consists of meeting all current requirements of establishing OQ’s through our three separate training providers: Energy World Net (EWN), Veriforce, and Industrial Training Services (ITS). New hires will be tested, and must pass, each module before being released to the field for field training. Once completed, the results for each module, and training provider, are documented and stored by the Field Operations Manager. LaSen, Inc. follows the Policy and Procedures outlined by the three training providers (EWN, Veriforce, and ITS) when establishing and maintaining the OQ’s for new hires.
Maintenance Process
It is the responsibility of the Field Operations Manager to establish the OQ’s for new hires and maintain those OQ’s through their validity. It is the responsibility of the Field Operations Manager to periodically audit the OQ’s for each Field Technician to keep them current and unexpired. Administrative duties and evaluation duties are the sole responsibility of the Field Operations Manager and will be carried out in due regard to the Policies and Procedures outlined by the three training providers: EWN, Veriforce, and ITS.
Audit Process
The audit process will be carried out and maintained by the Field Operations Manager and will consist of both digital recordkeeping, in the form of a spreadsheet, and hard copies of all OQ’s currently held by all Field Technicians, and their expiry dates. These records will be assessed on a periodic basis, but not too exceed 6 months. The Field Operations Manager will advise any Field Technician of any expiring OQ in frequencies of 60-day, 30-day, and 15-day notice. The Field Operations Manager will assign those OQ’s for evaluation no less than 30-days from expiration for renewal. Any Field Technician that does not renew within the allotted time will expire and be placed on suspension until such time as those OQ’s are renewed. No Field Technician will be allowed to perform aerial leak survey duties on LaSen’s behalf with expired Operator Qualifications. Field Technicians will also face additional disciplinary action, up to and including termination, for failing to renew OQ’s in a timely manner.
Span of Control
Span of control is defined as a management ratio, referring to the number of subordinates (pilot) controlled directly by a superior (Field Technician) while performing a covered task. LaSen, Inc. operates its OQ Plan under Span of Control as set by the training provider in conjunction with the Pipeline Operators. The following is a breakdown of Span of Control for each provider/operator:
Veriforce – 1:3
EWN – 1:1
ITS – 1:1
Note - LaSen, Inc. will not operate in conditions that require a 1:0 span of control as both pilot and technician are performing leak survey tasks above the ROW.
Covered OQ Tasks and Training
The following are the training provider covered tasks that LaSen, Inc. currently evaluates and maintains for its Field Technicians. It is worth noting again, that this list should not be considered all-inclusive. If an operator requests an OQ not listed in the following appendices, LaSen, Inc. will establish and maintain that OQ in addendum, in order to comply with the Pipeline Operators OQ requirements.
